Member must sign a one year commitment for membership Member must notify the spa 1 month prior to intended cancellation date of plan If an appointment is scheduled and not cancelled within 24 hours of booking, member will be charged, the service for the month will be deducted from the plan While massage receipt can be provided, it is ultimately up to the insurance company to cover any service that is submitted to them A credit card must be provided for monthly payments If the credit card charge is declined, member is responsible for offering a different credit card in order to complete the monthly payments Should the member not provide a new credit card, services will be paused until a new credit card is provided By agreeing to these terms, the member agrees to submit payment for the annual membership in full until the annual contract is complete. If the member does not cancel his/her membership, Laya Wellness will automatically continue to charge payments after the first year of membership Member must submit his/her desire to withdraw from the membership in writing
